"complete the return section on the front side of this form" what does this mean? What information do i need to add? thanks!

To return an item purchased online to us by mail, please generate your pre-paid return label here for Sephora US. > www.sephora.com/returnlabel

For Sephora Canada, please generate your return label here. > http://returns.narvar.com/sephora/returns/?locale=en_ca

Simply fill out the return form located on your Order Summary and include this inside of your return. I believe this section will ask which product/sku is being returned, and the reason for the return.
