Lost Return Form

Hi! I would like to return an item that I bought from sephora.com but I can't find the packing slip / order summary & return form.  I know that I can print out the "order summary" and the prepaid return label for the box, but what about the return form where I indicate which item I'm returning and why? I can't seem to find that info on the website, can I please have a copy of it that I can fill out? Thanks!
