Unable to add sample to order

I can't add samples to order. It came up to say SKU has duplicated. This is on the app. I tried web browser too but it redirect back to app. I tried restarting my phone. I tried uninstall and reinstall. Nothing works. This happens over the course of a month now. Does anyone else experience this?
