Survey QR code not working

Hi! I just visited one of my local Sephora stores and wanted to complete the survey, but the QR code fails to scan and the only email addresses which appear on my receipt are the standard Sephora.com, the FAQ regarding gift cards, and the returns and exchanges policy addresses. I even tried to manually type in my browser survey.medallia.com/sephora/usa and that only yielded an error message from Medallia that it couldn’t find my survey….

I submitted my sweepstakes entry, but I also wanted to provide feedback on the associate (Amber) who helped me yesterday at the Sephora located within the Mall of Millenia (store no. 796) and who provided stellar service while I made a perfume purchase (#Boy Smells Hinoki Fantôme).
She was so personable, professional, and enthusiastic, and I observed her consistency in being just as helpful and courteous to the customer before me. She was even able to help me skip the rather lengthy line to complete my purchase! I wanted to recognize her and show my appreciation for her efforts.
