Foundation separation

My foundation keeps separating even a couple hours after applying! Mainly at my nose and cheek bones. 

I moisturize PM and AM.  I use MILK hydrogrip primer (sometimes ELF poreless putty primer), make up forever redness corrector, Fenty beauty pro filt’r, CT airbrush flawless finish (sometimes Laura Mercier translucent setting powder). 

what can I do to keep this from happening? Different products? Thank you!
