Return Through Mail

I received my order in the mail and decided to return a couple things. However, I only received half of my receipt (at the top it says page 2 of 2 and I didn't receive the first page). Therefore, I am not sure how I will go about mailing back my return since I can't fill out the reason code, qty, etc.
