Waiver Authorization letter

I'm extremely upset with both fedex and sephora. First I had a package stolen which was nobody's fault however fed-ex didn't deliver to the correct address. They delivered the package outside of the apt complex building rather than delivering to my door and or taking it back to either be picked up or redelivery. Sephora did re-send to a friend's address who said wasn't delivered. So now sephora is telling me to contact fed-ex. Fed-ex said they closed the claim call the shipper (sephora). And it's a never ending cycle. Truly it's not sephoras fault (even thought they're using that carrier) fed-ex is the one who is delivering so they need to be better at delivering to the correct address and or taking it back to the facility to be re-delivered or picked up so things like this don't happen. The only reason why I'm upset with sephora is there unwillinglyness to assist. I was told sephora needed to submit a claim with fedex. Sephora "claimed" they did, but they couldn't provide me with the confirmation # for whatever reason (sounds sketchy), which fed-ex asked for. Not sure why they couldn't contact each other rather than having me contacting them 2-3x each. Regardless now I'm submitting a claim personally with fed ex which they're asking for "waiver authorization form". Please someome direct me to this documentation so I can be done with this and close my account. I'll stick with ulta from here on out. Never seem to have issues with them. Defienelty wouldn't recommend anything with sephora.
