URGENT!! W2 Former Employee

To whom it may concern,

I worked at Sephora from 2018-2019, and have still yet to receive my 2019 W2. I've contacted my store and after multiple attempts they finally sent me a form. I was told to fill it out and email it to what I guess was someone at HR? I sent this email with all the required documents 3 weeks ago and have heard nothing back. 

Taxes are due next week, and I really need my W2. I'm not sure what else to do. Please can someone get me in touch with someone who can help me?

I'm sorry to hear about this! I recommend reaching out to the Store Director at the store you were employed at so they can provide you with HR's contact information. If they are unable to assist, please send me a PM.
