Order Cancelled

MONEY OUT OF ACCOUNT BUT ORDER CANCELLED 

I have tried to order twice and both times my order has been cancelled  saying "we were unable to confirm your payment information" but the money has been taken out of my account from the second order.  My order number is #---------- and I am getting extremely annoyed with my order being cancelled each time. Please tell me and why this is happening, All my details are correct.
