Missing Item from Order

Hello, 

 

My order placed on November 25th (received December 1st) is missing the Kat Von D trial size Tattoo Eyeliner I purchased using 100 Beauty Insider Points shown below. This has never happened to me before. Can someone please help? Thank you!
