Unable to scan QR code for store survey

I have a QR  code from in store and am unable to scan it.  The ink is poor.  Will you please provide me with the site address fir the survey?  I had posted the site address on The Beauty PSA Thread but it doesn't seem to be working. 

Pls let corporate know that it'd be appreciated if the address was printed on the receipt.   Also, I've never had success with the QR codes.

Pls make sure you tag my correct username..look for the Krupp diamond!!! 💎

@keelybt when I click on the link, it says I've been entered in the sweeps.  Is that true?  My QR code won't scan again.

@LaurieBT thanks for the link.  It just prompted me to enter my contact info and said I was entered.  Will the entry really go thru if I didn't take the survey?
