Survey not working

I was at my local Sephora yesterday and the girl at the cash gave a piece of paper that said “We would like to hear from you” with the store number and her name. She said if I go to survey.medallia.com/Sephora/Canada and take the survey I can win a store certificate for $250… that website does not have a survey for Sephora or it’s very difficult to find. Can someone send me the link please or explain why this is not working
