Store Visit Survey Not Working

Had the most amazing experience when I went into the store at Sephora. Used to go to Ulta but decided to try Sephora instead and I have no way of getting the qr code to work to share details and name of associate who helped me get the map, scan my skin and match up the makeup. Since then I have made a few more purchases but really look forward to the chance to enter for the $250 gift card. 

Tried looking up from other community members but see similar issues. Any suggestions.
