Sephora Store Visit Survey Not Working

I went to Sephora recently and was shade-matched by an employee who worked efficiently and was so friendly.  I wanted to submit a store survey that iss at the bottom of the receipt, but it just kept routing me to the Sephora app's home page.  Can I get assistance please?
