Sephora QR code for Survey

My QR code form my receipt from an in-store purchase is not working for me to complete survey for a chance to win the $250 sweepstakes giveaway. How can I enter this drawing if my QR code doesnโ€™t take me there? Are there any other options? Thank you in advance!
