Order cancelled unsuccessful unauthorisation

My order got cancelled and it says unsuccessful authorisation? Why is happening? 😞

Yep I have had this issue going on for about 2 months; I have called at least 15 times, talked to 3 managers and I thought the issue was resolved only to have it happen again.
