Hacked?

Strange activity has been happening on my account for the past year. I have chose to ignore it for the most part but seeing as it is still happening I feel as if i should address it. 

First off, My birthday on my account has been changed from January 31, 2001 (my actual birthday) to June 5, 2001. I do not know if that was the "hacker" seeings as nothing else on my account has changed such as my email, password, or address. 

Second, for the past year there has been in-store purchases from different parts of the world. I live in Canada, but i have seen activity on my account from "Sephora Maine Mall", "JC Penny Mall of New Hampshir", "Sephora Cape Cod", and "JC Penny Steeplegate Mall", activity like new beauty insider card number associated, welcome kits redeemed, rewards being redeemed from my points, and store purchases.   

Could anyone give me some insight as to what I should do?

if i were i your situation I would contact customer support to see what they can do. sorry that you have to deal with this.
