Former Employee needing W2

I am in need of my 2022 W2 Form from the time I worked at Sephoa.  I have reached out several times and have been sent information from Fidelity for tax info but it does not have my W-2, just retirmenet account information.  Please help!

I have tried the directions that you gave me and it is not recognizing it.  I have tried "forgot username"  but it didn't send me the e-mail.
