Employee survey with emailed receipt

I would love to do the employee survey from my purchase today, but I opted for the emailed receipt. So I do not know where to find the 1, 2, 3, and 4.

I also am having trouble. I opted for an email receipt and there is a link but I can’t see the 4 sections on the email receipt. I hope all these comments prompt sephora to include instructions for email receipts as well.
