Amika mirrorball shampoo recall

Just received an email from Sephora regarding the Amika recall. I bought my shampoo from Sephora, how should I request for refund?

@Shahrin14, if your purchase was made more than 60 days ago, please fill out the request form on the Amika site for assistance with a refund: https://www.rqa-inc.com/client/BanskCons/

 

It looks like your private messages are turned off, and you can adjust your preferences in your Community settings (on the browser only, not the app). If your purchase was made within the last 60 days, you may send me a private message with your order number, and I'll be happy to assist. If the purchase was made in store within the last 60 days, the item can be returned in store. Thank you!

I was looking for information as well. I purchased this product from Sephora, much longer than 60 days ago. I visited the link you supplied but I only have a record of the purchase, I no longer have the product so I cannot take a photo.

@rihi, you may also contact their toll-free number 888-262-3713 Monday – Friday 9 am to 5:30 pm EST or email mirrorball@rqa-inc.com and reference amika mirrorball high shine + protect antioxidant shampoo for assistance.

Someone posted in the community form 2 months to which Sephora responded. Why is Sephora waiting two months to inform their customers of the recalll, you are putting people at risk
