Foundation Peeling off Face - Dry Skin

Hi there! I’m new to this community and am really hoping someone might be able to help me out!

I have dry skin, and notice that regardless of all the moisturizing i put on my face prior to makeup, once I put on my foundation, it starts peeling right off and separating.  I’m not sure it this is the foundation, primer, my skin, or that the products just don’t mix together. Has anyone experienced this with drier skin?

I currently use a Cerave hydrating face wash, a Youth To The People Mandelic Acid + Superfood Unity Exfoliant 4 oz/ 118 mL, Drunk Elephant Lala Retro™ Nourishing Whipped Refillable Moisturizer 1.69 oz/ 50 mL
On my face, I use either the Milk hydro primer, or the Tatcha primer, and the Nars Natural Radiant foundation. For concealer I use Born This Way. 

Any help would be greatly appreciated!!!

@CatalinaHernand I have dry/combo skin and don't use any of the products you mentioned.  I know I have tried the Milk hydro primer and the Tatcha one but they were not favorites for me.
