Base tips!!

hey everybody! i'm kind of new to the community, and i need help with my base.

i currently don't use primer, but i do use a moisturizer before applying. whenever i use foundation or concealer in certain areas, it cakes up, separates, dries up, etc. this happens in my t zone and around my mouth/chin. i want to use the nars radiant creamy concealer, if that's relevant. i really want to have a good base that lasts, i will take any tips!!

i have combination skin, sometimes dry sometimes oily. im mainly looking for a blurring primer, but im not that picky.
