Someone else using my account?

I have a few in store purchases appearing in my account from different locations but none of them were mine. I have also received the e-receipt for one. How worried should I be and what do I do about this?

This has been happening to me as well. It is so strange. It happened during the time we get the 20% discount and just now in the month of April while I was on vacation, someone used hundreds of my points. I am so annoyed. I changed my password and email on the account.
