If I had to suffer by only bringing along one lip product, I would definitely choose my Neutrogena MoistureShine Lip Soother. It's not a balm, but it is as moisturizing (possibly more) than one! This cooling gel provides you with sheer color and high shine. Plus, it contains SPF 20 to shield your lips from the sun's rays. My lips always feel refreshed and supple while and after using this amazing gloss! My absolute favorite shade is Gleam, which is a shimmery coral rose.
If I had to choose, my hands down choice would be Neutrogena MoistureShine Lip Soother. It's not technically a lip balm, and it qualifies as a gloss, but it has the same moisturizing properties as a lip balm does. And it comes in super cute colors. I love Sparkle, it's an ultra sheer baby pink gloss with shimmer. Plus, assuming the desert will be sunny, this product has SPF 20, so my lips will be protected from the desert suns harsh rays.
I'd have to pick Make Up For Ever Glossy Full Couleur because it's so thick and moisturizing. I can put it on and not have to reapply for hours. It is a bit sticky, but on a desert island I don't think I'd care about that.
I'm currently in love with Too-Faced Glamour Gloss! It's minty, long lasting, moisturizing enough to keep lips hydrated, and slightly plumping for a juicy, just-bitten look. On a desert island I think I would go for shades Champagne Room, First Time, or Plush.

Hmm...the lipstick I reach for the most often is Laura Mercier Shimmer in Amethyst...the close second would be NARS Cruising.
For gloss, my favorite is Fresh Sugar Lip Gloss (in Sugar Angel, now discontinued) -- it's hydrating and makes your lips smooth!
That easy..... I totally can't go without Lancome's La Laque Fever Gloss. It totally lasts all day, is not sticky (which is really important on a windy desert island), has an awesome shine , and comes in great colors.
Tough question! I would have to go with Korres Cherry Full Color Gloss. It gives just enough color and is hydrating enough to work as a lip moisturizer without actually being classified as one.
