HELLO!! I have a list for the perfect concealer for me and I was wondering if it exist!!! Here it is:
Creamy
Covers blemishes, hyperpigmentation, and under eye well
Has great yellow/golden undertones (i'm an NC40/ 153 in MUFE HD)
Works well with oily skin
No creasing
Doesn't need to be packed on
Feels light
No ingredients that will break me out

I love MUFE 5 Camouflage Cream Palette. It's creamy and I have dark circles under my eyes and it works well. Another good one is MAC Pro-Longwear concealer. I like MUFE a little better because you can mix a few of the colors to make your own. But either are great! The MAC is cheaper it's only $17 and MUFE is $36

I recommend Sephora's buildable cover kit. I bought the buildable concealer on its own in a Sephora inside JCPenney once but it is only available now as a part of this kit. As someone with very sensitive skin (and who often doesn't sleep as much as she should...), this concealer was great because of its creamy texture and total coverage without even needing to "build" it on as the name implies.

The only concealer I have ever used that might fit the bill for all of the issues you mentioned (blemishes, under eye, etc.) is TheBalm's TimeBalm concealer. It's very creamy and blendable -- it works well under the eyes, and I have used it on a blemish in a pinch.
However -- most of the time, what works well on a blemish isn't really suitable for the under eye area. I personally prefer thicker, dry-ish consistency concealers for blemishes -- such as NARS or Make Up For Ever Full Cover Concealer. A dry concealer will have good staying power on a blemish, and the thickness really helps it to cover without looking cakey (whereas with a thinner concealer, it's tempting to layer it in order to get adequate coverage). Under the eye area, however, you'll want a concealer to be creamer and more emollient; the under eye skin produces very little moisture, which is why so many concealers tend to cake look dry under the eyes. With this in mind, I really like TimeBalm or Benefit Erase Paste -- both are very creamy and non-drying, so they won't make the skin look dry.

I'm really loving Miracle Skin Transformer's Treat and Conceal. A little goes a LONG way (seriously. the tiniest dot will conceal my whole face), coverage is easily buildable, and it dries to a matte finish that will last all day (and that's hard work for face makeup for me. I work in a cafe and am preparing food in ovens and such all day).

I really like MUFE's HD Invisible Cover Concealer. I'm not sure if that would break you out but it's oil-free. The texture and coverage is pretty good, it's described as "medium".
I just tried out Clinque's Acne Solutions Clearing Concealer, and it helped heal my breakout. It is also oil-free and has a simiar texture and coverage as the MUFE concealer.
